Treatment of mantle cell lymphoma is difficult due to lack of generally accepted standards for initial therapy. First-line chemotherapy even with rituximab is not curative despite providing frequent remissions of the disease. Therefore, remission consolidation strategies are mandatory for majority of patients. Remission consolidation with autologous hematopoietic stem cell transplantation improves the progression free survival, but is feasible only in a limited proportion of patients due to median age of diagnosis being 63 years. Patients ineligible for transplantation can benefit from radioimmunotherapy that is a valuable consolidative strategy. In this manuscript a comprehensive approach to initial treatment of mantle cell lymphoma is provided.
