The below presented clinical case description concerns a long-lasting smoker with airway obstruction and then identified in our centre inoperable squamous carcinoma in the area of the tracheal bifurcation (cT4N2M0). The work presents a diagnostic-therapeutic scheme applied in a patient with central tumour, in which an indirect diagnostic role was played by the classical spirometry test with bronchodilatatory test carried out in an out-patient clinic.
